Social Prescribing is a pathway from your doctor or nurse practitioner’s office to local, non-clinical services for people who could use a little help connecting to their community or learning how to take care of their health and wellbeing. The social prescribing program has been adapted like many other community services due to COVID-19 however there are still many ways we can provide you with support to meet health goals, connect with community resources and maintain your wellbeing throughout this challenging time.
